Bespoke is run by a group of young ophthalmologists who started their own practices and who now use their collective knowledge and experience to help younger physicians start their own shops & grow them into power houses.

Words of caution

Opening an ophthalmology practice is one of the scariest and most daunting tasks you will ever undertake. But once you take the plunge, you’ll never look back…as long are you are mentally prepared for long hours, lean years, far more administrative headaches then you care to undertake, and all the hassles that come with being a business owner.

To be successful you will have to sweep floors, fix printers, train staff that leave as soon as they get good, and making uncomfortable decisions at every turn.

You will be the first person in the office each day and the last to leave and you will have to accept that no one cares about your practice as much as you do (except maybe us).

But like an oncoming train, however slow to get underway, once success comes, it will adapt an inexorable pace.

You will earn more than all your friends, even fully vested partners. You will be a top 1% earner, yet as your income rises your tax burden will decrease. The longer you stay in the game, the more assets you acquire, the less your efforts will directly affect the financial performance of your practice. This will give you a smooth inevitable off-ramp that affords you more freedom then even the most generous employer could allow. You will retain all the prestige and remain in control of your destiny.

It will be an exciting, anxiety-provoking life changing few years…but we wouldn’t trade it for anything! And neither will you.

Choosing a Practice location

Bespoke focuses on building businesses for our clients that deliver the lifestyles Millennial value. 

This means practices with very high degrees of automation, minimal staff, low overhead, robust E-Commerce functionality, and consumer focused marketing strategies.

All our new client consultations start with a map. We ask you to pick an intersection or street corner in a part of the country where you are sure you want to be. From there, we do the rest.

To the extent possible, we will find a practice location within walking or biking distance of your home, keeping your commute times to a minimum and increasing the efficiency with which you live your life!

At Bespoke, we value radical transparency and practice open book accounting. 

While you focus on working FT elsewhere to maximize your earnings, we will start your practice by doing the following…

  • Find a practice location close to your desired place of residence.
  • Prepare a pro-forma detailing costs of converting this space (or building new) into a beautiful medical office.
  • Procure financing for your project either internally from our Board of Directors, from a coalition of equity partners or a commercial lender. In rare cases we can also secure financing from the SBA but things rarely come to that.  
  • Design a space that fits your budget and aesthetic desires.
  • Select, source, and install flooring, ceiling, tiling, walls, reception desks, furniture and so on followed by all ophthalmic equipment (purchased at discounted rates over MSRP). 
  • After overseeing construction and outfitting of your new space we will procure a certificate of occupancy from the local municipality, satisfying all permitting requirements followed by installation of a robust clinic software architecture that include hardwired internet,  bookkeeping software, an EMR of your choosing, time tracking services, financial transaction software, and much more. All the articles of software required to run your practice are included.
  • Our credentialing experts will complete all requisite paperwork for you  including Type I & II NPIs, Tax IDs, DEA, CDS, etc. We also credential your practice with local payors, and aggressively negotiate excellent rates on your behalf. 
  • Once you open, we shift to a practice management roleThis means we can manage your billing, inventory management, act as a staffing agency supplying your practice with any and all staff and staff training required and direct your marketing efforts, including outreach to optometrists and fellow eye care practitioners in your area. 
    • These ongoing activities occur depending on your preferences only; meaning you are under no obligation to retain Bespoke on an ongoing basis and remain free to bring these services in house if you so desire.

Our Fee Structure

Bespoke Ophthalmics operates the way our founders practice ophthalmology–with complete price transparency.

At the start of a project we list all anticipated costs for site selection, environmental studies, permitting, design/build, equipment purchasing and installation, staff training, etc.

These costs are reviewed with you to assess project feasibility and at this stage painful choices may need to be made. But Bespoke will be with you from the first choice to the last.

For our time, we assess a profit per project that is listed as a separate line item and which never changes for any reason. 

The project then proceeds with Open Book Accounting so you can audit our books at any time to determine where every penny is being spent and veto any decision. Its your practice…if you want moldy carpet installed, you can have it!

Since we internally finance our projects and cost overruns don’t improve our profits, we have every incentive to complete your project on time and underbudget.

Bespoke never seeks equity in our practices. Like you, we are entrepreneurs and want to move onto our next project, leaving you to grow a beautiful business in your own time!
